To Make An Appointment

New patients:
A referral from your primary care doctor is required, even if your insurance doesn’t require one. Please have your primary care doctor’s office contact us to initiate the process.

Returning patients:
If your return appointment is greater than 3 months out, we will contact you by phone or a postcard will be mailed to you one month prior to your return date as a reminder to make an appointment.

Appointments less than three months away are scheduled while you are at the clinic.

If you are having symptoms or feel that you need to be seen, please call the clinic. If you are having chest pain or feel there is an emergency, please call 911 immediately.
