To Make An Appointment
New patients:
A referral from your primary care doctor is required, even if your insurance doesn’t require one. Please have your primary care doctor’s office contact us to initiate the process.
Returning patients:
If your return appointment is greater than 3 months out, we will contact you by phone or a postcard will be mailed to you one month prior to your return date as a reminder to make an appointment.
Appointments less than three months away are scheduled while you are at our clinic.
If you are having symptoms or feel that you need to be seen, please call our clinic. If you are having chest pain or feel there is an emergency, please call 911 immediately.
Missed Appointment Policy
Our goal is to provide quality medical care in a timely manner. No-shows, late arrivals, and cancellations inconvenience those individuals who need access to medical care. We would like to remind you of our policy regarding missed appointments.
Cancellation of an Appointment
In order to be respectful of the medical needs of other patients, please be courteous and call the office promptly if you are unable to show up for an appointment. If it is necessary to cancel your scheduled appointment, we request that you call at least 48 hours in advance. Your early cancellation will allow another patient access to timely medical care.
How to Cancel Your Appointment
To cancel your appointment, please call (503) 485-4787. If you do not reach the receptionist, you may leave a detailed message on our voicemail which is checked daily. If you would like to reschedule your appointment, please leave your name and phone number so a team member may reach out to you.
No Show Policy
A “no-show” is a patient who misses an appointment without cancelling it. Failure to arrive at the time of your scheduled appointment will be recorded in the patient’s chart as a “no-show”.
**The first time there is a “no-show” there will be no charge to the patient. A 2nd occurrence will result in a $25 fee. The 3rd occurrence will be a $25 fee and the possibility of being discharged from the practice.**
